If you or a loved one have been diagnosed with a life-limiting condition, helping your family is more than just a mission for Amazing Grace Hospice™—it’s our passion to deliver care that addresses your physical, emotional, mental and spiritual needs at this significant time of life. Our team is ready to provide comprehensive hospice services to every person in our community who needs and desires them.

Our Services

  • In Home Care

Most patients and families want to receive care in the comfort of their own home.  In more than 90 percent of Hospice care cases, services are delivered in patients’ homes and/or in settings such as community-based residential facilities, assisted living facilities and nursing homes.

  • Inpatient Care

When in-home care cannot meet the needs of our patients and families, contracting inpatient care with hospitals and skilled nursing facilities may be an option.  Inpatient care provides short-term, intensive monitoring and treatment of acute pain and other complex symptoms.  Amazing Grace Hospice also understands that families may need respite care to deal with their loved one undergoing hospice care, and we are glad to offer respite care when needed by the family.

  • Respite Care

Being a caregiver for a loved one or friend is a full-time job. No one is equipped to do it alone. As a caregiver, it’s important to take good care of yourself so you can provide the best possible care for your loved one. Respite care provides a temporary break in care giving responsibilities. Respite care comes in different forms that are suitable for various needs. Respite programs provide planned short-term and time-limited breaks for families and caregivers in order to support and maintain the primary care giving relationship. Respite provides the much needed temporary break from the often exhausting challenges faced by the caregiver.  Respite also provides a positive experience for the person receiving care.   Amazing Grace Hospice provides respite care through contracted facilities.  Respite care is so important that a provision for it is built into the Medicare Hospice Benefit.

  • Patient Care Coordination

We provide coordination of care for patients being looked after at home.  These services may include appointments to see a consultant, regular visits by registered nurses, hospice aides, social workers, a Pastor/Chaplain, therapists, or other support services provided by hospice.

  • Bereavement Support

When someone in the family is diagnosed with a life-limiting illness, everyone in the family is affected. Our care service extends beyond the treatment of physical symptoms to the emotional and social needs of patients and their families.  We team up with qualified and experienced counselors who help people deal with the emotional and psychological consequences of someone close to them dying.

We also have a monthly bereavement support group that is open to all members of the community, offering professional advice and encouragement to everyone who has experienced the loss of a loved one.

  • Social Worker Support

Amazing Grace Hospice works with a team of qualified and experienced Social workers who help support everyone from children to adults in facing the emotional and psychological consequences of someone close to them dying.
